AHO was organized in 2019, and established as a formal entity in 2023. AHO governance and operations structures are set forth in the organization's bylaws. A small Executive Office (President, Vice President, Secretary, Treasurer—presently vacant) leads the overall conduct of alliance business. However members and associated alliance affiliates have wide latitude to engage in bilateral and multilateral partnerships and joint projects as they deem appropriate.
An Executive Board provides governance oversight to the AHO Executive Office, and holds significant decision-making authority for the alliance on matters such as membership constructs and decisions.
Topical committees (e.g. Membership Committee) are established by board action, and are charged with advising the board and the executive office in selected matters of alliance business.
Finally General Assembly meetings are held to conduct alliance business and select board members and executive officers.
